Help Applying for Insurance

Capstone Rural Health Center has Certified Application Counselors (CAC) available and ready to assist with any of your insurance enrollment needs and questions. Assistance with Insurance Enrollment is a 100% FREE Service!

Certified Application Counselors are available at all Capstone locations. Walk-Ins Welcome, but that does not guarantee an appointment on that time or day.

What to Bring

  • Valid Photo ID
  • Proof of Income
  • Documentation must show hourly, weekly, bi-weekly, monthly, or annual income.
  • Tax forms are accepted.

 

What to Expect

You can expect a friendly, certified, insurance enrollment specialist to help you apply for Alabama Medicaid & ALLKIDS/CHIP, Medicare, or the Affordable Care Act (also known as ObamaCare). You may qualify for a Special Enrollment Period outside Annual Open Enrollment.

The application process can typically take up to 30-45 minutes.

Approval or denial for either program is granted by Alabama Medicaid and the Health Insurance Marketplace, and not by the Certified Application Counselor.
