Capstone Rural Health Center has Certified Application Counselors (CAC) available and ready to assist with any of your insurance enrollment
needs and questions. Assistance with Insurance Enrollment is a 100% FREE Service!
Certified Application Counselors are available at all Capstone locations. Walk-Ins Welcome, but that does not guarantee an appointment
on that time or day.

What to Bring
- Valid Photo ID
- Proof of Income
- Documentation must show hourly, weekly, bi-weekly, monthly, or annual income.
- Tax forms are accepted.
What to Expect
You can expect a friendly, certified, insurance enrollment specialist to help you apply for Alabama Medicaid & ALLKIDS/CHIP, Medicare, or the Affordable Care Act (also known as ObamaCare). You may qualify for a Special Enrollment Period outside Annual Open Enrollment.
The application process can typically take up to 30-45 minutes.
Approval or denial for either program is granted by Alabama Medicaid and the Health Insurance Marketplace, and not by the Certified
