Transportation
Capstone Health Center provides transportation services to appointments for Capstone patients. Handicap & wheelchair accessibility available. Patients must have a scheduled appointment at a Capstone Health location and be placed on the transportation schedule to reserve a spot for patient transport. A liability waiver form must be signed by the patient to be eligible for transportation services.

Capstone Health has a passionate Outreach Team who serves our patients and educates the community about our services through outreach activities in community, hosting and participatin in health fairs, partnering with agencies with similar issions, and providing resource specific to food assistance, utility assistance, and houseing assistance.
