New Patients
Capstone Health Center is always accepting new patients with Commercial & Private Insurances, Medicare, Medicaid and Uninsured. Capstone provides all levels of primary & preventative health care services. Patients should arrive 30 minutes early to fill out all paperwork. Co-payments or Nominal Fee are due at time of service.
All patients should bring the following items to their appointment:
- Valid Photo ID
- Insurance Card(s)
- Sliding Fee Discount Program Documentation
- Proof of Residency
- Co-Payment/Nominal Fee
- Current Medications
