Job Description
Capstone Health seeks a Certified Family Nurse Practitioner to join our team. The ideal candidate is service orientated, and patient driven, enjoys working with underserved patients and is culturally sensitive. Current licensure in the State of Alabama and Board Certification/Eligibility required. Primary duties will consist of treating patients in the clinic and telehealth visits. Occasional evening, weekend, or after-hours work may be required to support patient care and call coverage.

Minimum Qualifications
EDUCATION
1. Master's or Doctoral degree in Nursing from an accredited Nurse Practitioner program.
2. Current unrestricted Registered Nurse (RN) license in the State of Alabama.
LICENSURE & CERTIFICATION
1. Current Alabama Advanced Practice Registered Nurse (APRN) license.
2. National certification appropriate to the assigned specialty (e.g., Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (AGNP), Psychiatric Mental Health Nurse Practitioner (PMHNP)).
3. Current DEA registration, if prescribing controlled substances.
4. Current Alabama Qualified Alabama Controlled Substances Certificate (QACSC), if applicable.
5. Current Basic Life Support (BLS) certification.
6. Advanced Cardiac Life Support (ACLS), when appropriate to the practice setting, preferred.
EXPERIENCE
1. Minimum of two (2) years of clinical Nurse Practitioner experience or equivalent clinical experience appropriate to the assigned specialty.
2. Experience in a Federally Qualified Health Center (FQHC), community health center, or primary care setting.
3. Experience with Electronic Medical Record (EMR) systems, telehealth services, and working with interdisciplinary teams.
KNOWLEDGE, SKILLS, ABILITIES
1. Knowledge of primary care, preventive medicine, chronic disease management, and evidence-based clinical practice.
2. Knowledge of applicable federal and state laws, regulations, and professional standards, including HIPAA, OSHA, HRSA Health Center Program requirements, collaborative practice requirements, and organizational policies.
3. Ability to perform comprehensive health assessments, diagnose medical conditions, develop treatment plans, and provide patient-centered care.
4. Strong clinical judgment, critical thinking, diagnostic, and problem-solving skills.
5. Excellent verbal, written, and interpersonal communication skills with the ability to establish effective relationships with patients, families, and healthcare team members.
6. Ability to effectively manage urgent and emergent clinical situations within the scope of practice.
7. Strong organizational and time management skills with the ability to prioritize multiple clinical responsibilities.
8. Proficiency with Electronic Medical Record (EMR) systems, clinical documentation, and standard computer applications.
9. Ability to collaborate effectively with physicians, nurses, behavioral health providers, pharmacists, care coordinators, and other interdisciplinary team members.
10. Commitment to continuous learning, quality improvement, patient safety, and professional development.

1. Ability to remain stationary for extended periods while evaluating patients and completing documentation.
2. Ability to move throughout clinic locations to provide patient care.
3. Ability to communicate effectively in person, electronically, and by telephone.
4. Ability to operate standard office equipment, clinical equipment, and computer systems.
5. Occasionally lift or carry medical supplies or equipment weighing up to 20 pounds.
